Both are fiords carved by glaciers on the same stretch of the South Island's south-west coast, and both are in Fiordland National Park. They are not interchangeable, and the price gap is not a quality gap — it's a transport gap.
Milford has a road to it. You drive, or a coach drives you, and you step onto the boat. That access is why a cruise starts at NZ$169, and also why you'll share the fiord with a lot of other people, particularly between about 11am and 2pm when the Queenstown day coaches converge.
Doubtful has no road. Getting there means a boat across Lake Manapouri, then a bus over the Wilmot Pass, then your cruise — and the same chain in reverse. That's the whole reason it costs NZ$359 and the whole reason it's empty. You're not paying for a better fiord. You're paying for the difficulty that keeps everyone else away.
If you can only do one and you've never seen either, do Milford. If you've done Milford, or the idea of a busy viewing deck defeats the purpose for you, do Doubtful.

Fiordland is one of the wettest inhabited places on earth, with Milford recording several metres of rain a year. Rain is the normal condition, not a ruined trip. Heavy rain produces hundreds of temporary waterfalls down the rock faces that aren't there on a dry day. Book expecting rain and treat a clear day as the exception.
It's NZ$749 per adult in a double or twin-share cabin against NZ$359 for the day cruise. The extra buys the part day visitors never get: the fiord after the day boats leave, kayak and tender excursions, dinner aboard, and the sound of the place at night. If you're choosing between two Fiordland trips, most people are better served by one Milford day cruise plus a Doubtful day cruise than by a single overnight.
The inner fiords are sheltered and usually calm. Milford cruises that run out to the Tasman Sea entrance can meet open-ocean swell for a short stretch at the mouth, which is where motion is noticeable. Doubtful is longer and generally more sheltered. If you're prone to motion sickness, take something before boarding rather than after.
